Being in-network with an insurance carrier does not guarantee all services provided will be covered by your insurance plan. The following is a list of affiliated providers from whom you may receive a bill for services. Independent providers involved in your treatment may or may not be in network with your insurance plan. We strongly recommend contact all providers involved in your care prior to your procedure.

Participating Healthcare Plans and Affiliations
It is important to note that insurance plans differ by employer, subscriber, deductible preference, and/or affordability. While your insurance carrier will tell you that it is your responsibility to know your plan, the Center is prepared to help you navigate through these complicated nuances.
Please understand that no question is a bad question and we will gratefully assist you with your concerns.
Some insurance plans consider certain facilities in-network meaning that there are agreed upon charges for the surgical procedure or endoscopy. In cases where the Center has not agreed upon rates, or if the insurance plan is not administered locally, the Center would be considered “out-of-network”. In both cases you may have a deductible, co-insurance, or copayment due. You may have no out-of-network benefits and the procedure cost would not be covered by insurance. Please contact your insurance company with any questions regarding network participation or what will (or will not) be covered by your particular policy or plan. Deductibles, co-insurance, and co-pay amounts are set by your insurance plan. The Center is obligated by contract with your insurance plan to comply with collection of these fees.
Prior to your procedure, our staff will verify your insurance coverage in order to estimate your patient responsibility. You may be contacted by a Center representative to let you know your financial obligations. As noted above, depending on your coverage, you may have a co-pay, co-insurance or deductible. This obligation is due on the day of your procedure, unless other arrangements have been made in advance. The Center accepts Visa, MasterCard, American Express, Discover, Flexible Spending cards, personal and cashier’s checks, and cash. Your procedure may be rescheduled if payment is not made.
You may find it helpful to contact your insurance company prior to your procedure to better understand your coverage. Make sure you tell your insurance carrier that the Center is an outpatient ambulatory surgery facility to assure you receive the correct information.
The Center will bill your insurance for the facility fee. Your surgeon, anesthesiologist, and pathology laboratory will bill separately for their services.
